Gerhardt's sign is observed in thrombosis of the cerebral transverse venous sinus (sinus transversus seu sigmoideus) and is manifested by a lesser filling of the external jugular vein (vena jugularis externa) on the affected side compared to the healthy side, since on the side of the occluded sinus, blood from the external jugular vein more easily penetrates into the internal jugular vein (vena jugularis interna).
