Summary

Hydropyonephrosis is the transition of hydronephrosis into pyonephrosis due to infection of the hydronephrotic sac. Infection typically spreads via an ascending route, or hematogenously, lymphogenously, and sometimes through catheterization or pyelography. Treatment is exclusively surgical, consisting of nephrectomy if the contralateral kidney is healthy, or nephrostomy otherwise.

HYDROPYONEPHROSIS (from Greek hydor - water and pyon - pus), the transition of hydronephrosis into pyonephrosis as a result of infection entering the hydronephrotic sac. The pathways of the latter's penetration are: hematogenous, lymphogenous, and the most frequent - ascending. Sometimes infection of hydronephrosis occurs during ureteral catheterization or pyelography. Treatment is exclusively surgical - nephrectomy, if the second kidney is healthy; otherwise - nephrostomy.
